Current NHS screening criteria for women under 50 miss up to 95% of those at higher risk of developing breast cancer, according to researchers from Cambridge University and the Institute of Cancer Research, who warn the guidelines are too narrow.
Under existing rules set by the National Institute for Health and Care Excellence (NICE), women under 50 receive no routine breast screening unless they have a strong family history of the disease. The guidelines focus solely on inherited genetic factors, ignoring lifestyle and other risk indicators.
The study, published in the British Journal of Cancer, used a risk-assessment calculator called Boadicea which factors in family history, lifestyle, reproductive history and genetic data. When applied to women under 50, the tool identified eight times as many individuals who subsequently developed breast cancer compared to the current NHS approach.
Researchers estimate that if all women under 50 received this comprehensive risk assessment, roughly a quarter (26.5%) would be categorised as above-average risk and referred for further screening — including a third (34.8%) of those who develop breast cancer within the next decade.
Breast cancer is the leading cause of death in women under 50 in the United Kingdom. About one in seven women will develop the disease in their lifetime, yet only 5–10% of cases are linked to inherited genes. Most cases (96%) occur in women over 40. Men can also develop breast cancer, with around 420 new cases diagnosed in the UK annually.
One Labour MP on the Commons Health Committee told the BBC it was time to consider mandatory childhood immunisations — drawing a parallel with vaccination debates — and called for a review of cancer screening policy to account for the wider risk factors the study identified.
